New Medicaid Bulletin Available - Tailored Care Management Update

DHHS previously released the draft Tailored Care Management Provider Manual and application questions for providers interested in becoming Advanced Medical Home Plus (AMH+) practices or Care Management Agencies (CMAs). Updates and edits were made based on responses and aggregated into the Memo on Tailored Care Management Updates.

However, as a result of the Public Health Emergency COVID-19, the Tailored Care Management certification process has been suspended. Find out the next steps here.
