Provider Permission Matrix (PPM) Update to Accreditation Type

Please note that the text "Nationally Accredited Entity" will be removed from the Accreditation Type column of the Provider Permission Matrix (PPM). Instead, rows under this column will indicate that one of the following accreditations are required:

• Joint Commission on Accreditation of Healthcare Organization (JCAHO)                              
• Council on Accreditation (COA)                                                                     
• Council on Quality and Leadership (CQL)                                                           
• Commission on Accreditation of Rehabilitation Facilities (CARF)
