Taxonomy Enrollment Requirements Updated
According to SL 2018-5 SB 99, effective July 1, 2018 select provider types (taxonomy codes) have changed from High Risk to Limited Categorical Risk. Due to this change, some taxonomy codes no longer require fingerprinting, a Federal Site Visit and/or the federal fee.

* This change applies to IDD waiver providers who are accredited. Personal Care Services (PCS) providers using these taxonomy codes must complete fingerprinting, site visit and pay the federal fee.
