Override Requests using the Medicaid Resolution Inquiry Form
Many of the override requests received by CSRA using the Medicaid Resolution Inquiry Form are missing the required claim form or include a claim copy which is not suitable for scanning. An original claim form should be attached to each override request submitted. Inquires which do not include an appropriate claim form cannot be processed. Requests must also include single-sided copies of documentation to support the override request.
