Dialysis Initial Treatment Date Must be Included

Providers are reminded that they must enter the initial treatment date on dialysis claims. If a dialysis claim is submitted without a First Treatment Date on the claim, the claim will deny. The Explanation of Benefits (EOB) that will post on the paper Remittance Advice (RA) is EOB 00189 - MISSING OR INVALID FIRST TREATMENT DATE FOR DIALYSIS CLAIM. PLEASE RESUBMIT WITH FIRST TREATMENT DATE.

Claims that deny must be resubmitted with the First Treatment Date to be processed.
