Update to Short- and Long-Acting Opioid Analgesics PA Forms

An update has been made to clarify the Short- and Long-Acting Opioid Analgesics prior approval (PA) forms on the NCTracks provider portal. The question on the forms regarding whether the PA request is for an initial authorization or reauthorization has been changed to match the online PA request screen. The question now reads:

"Is this an initial authorization request?  Select 'Yes' for an initial authorization.  Select 'No' for a reauthorization request."

Providers submitting PA requests for Short- or Long-Acting Opioid Analgesics should use the latest version of the forms, found on the Drug Request Forms page. Providers who are using a previously downloaded copy of the forms should discard the old version and download the new version.

Providers are encouraged to submit PA requests online via the portal to expedite evaluation of the request.
