The NADAC Pricing for the Zyvox 600mg tablet has been end dated. As a result, claims for Zyvox 600mg tablet are rejecting for PA required. Because this is a preferred brand, PAs are being provided for Brand Zyvox 600mg tablet claims (upon request) until the issue is resolved. Pharmacies or providers may request a PA for Brand Zyvox 600mg tablet.
