Attention: Pharmacy Providers and Prescribers Nucala (mepolizumab) Now Covered
Nucala is now covered by NC Medicaid and NC Health Choice with prior approval. Providers may request Nucala using the temporary PA forms and process below:
Providers must mail or fax the Prior Approval request using the Standard Drug Request Form as the first (top) page and add the Nucala PA Attachment behind it.
