Update on BH I/DD Tailored Plan Care Management

DHHS is hosting a Stakeholder webinar on Thursday, May 30, 2019, from 10a.m.-11a.m. This webinar will provide an overview of the planned approach to care management for beneficiaries enrolled in BH I/DD Tailored Plans, including:

• The vision for managing beneficiaries' total needs across medical, behavioral and unmet health-related resource needs through care management;
• An overview of BH I/DD Tailored Plans and provider roles and responsibilities under the model;
• Specific requirements of the model; and
• Relationships with waiver and other existing programs.
