AMH Tier Attestation Tool Training

The North Carolina Department of Health and Human Services (DHHS) developed the Advanced Medical Home (AMH) program as the primary vehicle for delivering local care management as the state transitions to Medicaid managed care. There are multiple AMH tiers. Providers will be grandfathered as AMH Tier 1 or Tier 2 providers. However, providers may attest to a high tier level using the AMH Tier Attestation Tool, which will be available on October 1, 2018.

An instructor-led training course is being offered via WebEx to assist providers in using the AMH Tier Attestation Tool. The first training session will be on Friday, September 14, 2018. For more information, including how to register, refer to the September 4 announcement.

New AMH Tier Attestation Tool Job Aid

In addition, a Job Aid is now available to guide providers on completing the Advanced Medical Home (AMH) Tier Attestation. To view the “Advanced Medical Home (AMH) Attestation” Job Aid, logon to the secure NCTracks Provider Portal and click Provider Training to access SkillPort. Open the folder labeled User Guides (reference library). The Job Aid can be found in the sub-folder labeled Reference Documents.

Refer to the Provider Training page of the public Provider Portal for specific instructions on how to use SkillPort. The Provider Training page also includes a quick reference about downloading Java, which is required for the use of SkillPort.
