New Prior Approval Page on NCTracks (with DMA PA Forms)

A new Prior Approval (PA) page has been added to the public NCTracks Provider Portal. It contains links to the latest N.C. Division of Medical Assistance (DMA) PA forms for submission to NCTracks, both Medical and Pharmacy. (The Pharmacy PA forms were already on the NCTracks website.) This list is now the definitive source for Prior Approval forms required by DMA.

The preferred method to submit prior approval requests is online via the secure NCTracks Provider Portal. However, providers can also submit paper forms via mail or fax.

Legacy DMA PA forms will be accepted through August 15, after which providers must use the NCTracks forms on the Prior Approval page. (Legacy PA forms submitted after August 15 will not be processed.)

Note: This does not affect prior approvals submitted to MedSolutions, CCME, or other vendors.
