Individual Provider Applications Returned for Request of Transcripts

Since the change requiring new information in applications for Individual Providers went into effect, some providers have had their applications returned with a request for school transcripts. Providers in this circumstance will receive the message below:

In order to process your application, we must verify your education. Contact [School Name] and request a transcript be emailed to ProvEnrollSuppData@gdit.com. If the school is unable to electronically send the transcript to an email address, request the school send a sealed transcript to: Provider Enrollment, PO Box 300009, Raleigh, NC 27622.

*Note: For Transcripts ONLY, please disregard the standardized text below with instructions to upload an electronic copy of the required documentation to the Provider Portal. The transcript is an exception and must be sent via certified mail or emailed from the school.

Due to the potential delay depending on schools returning the transcripts, an adjusted time frame will be offered for applications in this circumstance. Providers will initially have 30 days to submit the transcript. If NCTracks does not receive it within 30 days, another 30-day notice will be sent requesting the transcript, prompting the provider to follow up. This will allow a total of 60 days for receipt of the transcript. If the transcript has not been received within 60 days, the application will be abandoned and the provider must resubmit the application and pay all applicable fees.

As mentioned in the note above: School transcripts are an exception to the typical instruction of uploading the applicable documents on the Upload Documents page. The official nature of school transcripts requires them to be submitted via the secure email address or mail only; they may not be uploaded by the provider.
