Federal Enrollment Application Fee Increase for Calendar Year 2021
The federal fee for Medicaid enrollment has increased from $595 for calendar year (CY) 2020 to $599 for CY 2021.

The N.C. Division of Health Benefits (DHB) collects the federal fee as required under Section 1866(j)(2)(C)(i)(l) of the Affordable Care Act (ACA) from certain Medicaid and N.C. Health Choice (NCHC) providers. The Centers for Medicare & Medicaid Services (CMS) set the fee, which may be adjusted annually.
The federal fee is required for the following:
- Initial enrollment application
- Re-enrollment application
- Manage Change Request (MCR) to add a new site location
- Re-verification application
