DEA Designation Form Updated

The Drug Enforcement Administration (DEA) Designation Form has been updated to include an additional option to select for providers that are in an ACGME accredited training program and do not practice independently. The updated form may be found on the Provider Policies, Manuals, Guidelines and Forms page under Provider Forms. Providers should use this revised version when submitting an application.
