Submit FCBC Application, EFSRI, and Fingerprinting by Deadlines to Prevent Termination
Please note that when a Fingerprinting Criminal Background Check (FCBC) application request notification is received, the provider must submit the FCBC application within 30 days from receipt to avoid termination for cause. After submission of the FCBC application, the provider will receive a letter and the Electronic Fingerprint Submission Release of Information Form (EFSRI) informing them how to complete fingerprinting. If the EFSRI form is not uploaded to the record within 30 days, the provider will be terminated for cause.
