Monoclonal Antibodies Drug PA Criteria Consolidated

The prior approval (PA) criteria for Monoclonal Antibody drugs, including Nucala, Fasenra, and Xolair, have been combined into a single document on the Prior Approval Drugs and Criteria page of the NCTracks website. The separate criteria for Nucala has been removed.

However, individual drug forms are still being used for PA requests for each of the Monoclonal Antibody drugs, available on the Drug Request Forms page.
